​Higher COVID death among doctors- An Epidemiologist point of view

WTN- This scenario is due to doctors themselves, putting them at risk due to a lack of scientific foresight. When involved in policy decisions of how Pandemic should be controlled and the battle should be won, They fail to device effective protocols first to protect themselves against the risk of exposing them to too much of a chance of contracting the SARS-CoV-2. 

1. They, as experts, should have refused to treat asymptomatic patients. They are trained at a higher level then distributing paracetamol and checking fever. 

2. They should have been posted in a super-specialty ward and ICU where sick Patients were admitted with the need of oxygen and Invasive and non invasive ventilators.  

3 . Doctors should have been categorized based on IgG antibodies positive and negative. A positive test shows they have exposed, and some immunity for COVID -19 has developed. They can handle the needed close monitoring, patients' clinical examination, and crucial decision making for advanced therapies. I don't think we can be careless saying there own social distancing protocols will protect us and unlikely we will be positive. The Pandemic is already in the community spread stage. The government can keep saying in our country; there is no community spread.   They may do that for a reason to prevent the public panic reaction. All epidemiologist knows its community spread, so speak to them. A simple logistic analysis will even tell you that community spread has started long back. 

4. Doctors negative for IgG. They are not exposed to COVID-19 yet. They should only involve in remote monitoring. 

5. Even doctors with IgM negative, again, a serology lookout for T cell-based immunity can be assessed. In Singapore, 50% of the patients have T cell immunity, who were not even exposed to COVID. It is shown that this type of immunity is achieved through exposure to a similar group of viruses before. 

5. The young adult who is seropositive for IgG should be trained to manage asymptomatic patients, fever clinics, and do the groundwork. It is not cowardly. In war, the general remain behind the front line. When he loses his life, the battle is lost. 

This simple protocol can drastically reduce doctors' death.
